Once you have submitted your form we will email you to confirm we have received your application. If you do not receive this email, you may not have entered your email address correctly. Please call the Junior Kit Scheme Officer on 0845 345 4555 ext4274 for confirmation of receipt if you do not recive the email within 24 hours.

If your form is submitted successfully, you do not need to send us a printed version of your form as the electronic signatures are sufficient.
